COLLINS, TERESA LYNN, passed away on Wednesday, January 27, 2010.
She was a member of 600 Cerebral Palsy Club, Easter Seal Booster Club, and Open Door Baptist Church.
She was preceded in death by her grandparents, Arnold and Flora Casebolt, and uncles, Jack and Jimmy Lee Casebolt.
Survivors include her mother and stepfather, Mary Lou and Wayne Klein; her father, James E. Collins; and her brothers, Douglas and Dennis Klein.
Funeral services will be 7 p.m. Friday at Owen Funeral Home, 5317 Dixie Highway with burial in Rocky Fork Cemetery, Newark, OH.
Visitation will be 2-7 p.m. Friday.
