Thomas James Hennessey, IV, 31, Clarksville, died Thursday, October 15, 2009, at Vanderbilt Medical Center.
A memorial service will be held at 2 p.m. Sunday at Neal-Tarpley Chapel with Pastor Tommy Vallejos officiating.
Visitation will be from 11 a.m. until the time of the service Sunday at the funeral home at the funeral home.
The remains are to be cremated.

He was born July 10, 1978 in Tampa, Florida, son of Thomas James Hennessey, III; and Marta Rejina Hernandez Roberts.
He was a member of Faith Outreach Church. Thomas enjoyed Martial Arts where he earned the High Brown Belt. He was a Jr. Pro Football Coach and Soccer Coach.
Survivors include his parents; his wife, Cynthia Agravante Hennessey; son, Ethan James Hennessey; daughter, Mercedes Agravante Hennessey; brothers, Caleb Hennessey and Ted Reaume; and sister, Michele Lee Curtis.
