Devoted father of Jacalyn Moore and husband Michael of Revere, Debra Lanzillo and husband Paul of Saugus and Lisa Mullins and husband Frank of Saugus. Loving son of the late John and Bessie (Namet) Forman. Dear brother of the late Henry Forman and Jeanne Forman. Loving grandfather of Richard C. Lanzillo and fiancé Deanna Baranski, Derek Lanzillo and wife Katie and Bobby Mullins. Cherished great-grandfather of Layla Eve Lanzillo. US Marines Veteran. Served in the Korean Conflict. Services at the Torf Funeral Chapel, 151 Washington Avenue, Cary Sq, Chelsea, MA on Thursday, April 13th at 10AM. Interment in Agudas Shalom Cemetery, Everett. Shiva will be held immediately following the burial until 7PM, resuming Friday from 1-5PM and Saturday from 5-7PM at the home of Jacalyn and Michael Moore.
